Error in applyFBSBS

I have a Dreamweaver site with php. I open a php page, then open WEB ASSIST: SECURITY ASSIST: CREATE SECURITY PAGES. I choose a data connection, which I click on EDIT and TEST to make sure I get confirmation that the connection is OK. No matter what options I choose in the wizard, including pure defaults (and I been at this for 9 hours now), when the wizard runs it gets part way through creating the login.php page when it hangs and displays:
Error in applyFBSBS:
Error: nodeToOffsets: Argument number 1 is invalid.
It also displays an OK button. I click OK, and everything continues to hang. No user registration page or update page or any other .php pages are created. I've read the security assist documentation and followed the instructions and the wizard many times. I can find no mention of this error message. How can I make Security Assist work?
