Insert upload path to database

Hi, i created a form where an image can be uploaded.
With Digitial File Pro i created it in a way that it will use the name formfield as being the name of the upload folder by choosing the lightning bolt and selecting the form field for the name.
I did the same for the filename. So this will cause the image to end up in the folder named as specified in the form name field and the image file will have the same name as well.

All works fine except that the dataassist insert behavior at this moment only inserts the correct filename. I want to insert the folder and filename as a whole path in the database like: foldername/filename
In my case both will have the same name like: testname/testname.jpg

Is this possible to do with the DataAssist insert behavior and if yes what to choose and how to combine both? I now choose the Server_name and it will insert only the name currently.

Thanks
