insert doesnt seem to work as a plugin call

Hi there, ok this is odd (it worked before) but for some reason its stopped.

in the zip file I have included the main file causing problems and some additional files in case you need to check them too.

(Im only using the plugin feature for Framework builder)
(a further note, things like the login system on the template works fine)

I have a template thats made up of a series of plugins. then I use this template to create the main page (create-account.php - which then calls another plugin that is just a basic form and inserts data into the database. (this plugin works fines if i run it by itself in the browser)

However, if i call this plugin from the create-account.php file then the insert into the database doesnt work and the browsers shows the contents of my plugin folder.

What could be wrong? as the login system works fine (and im using the same method of page construction with that).
