2.09 issue with Single Record insert

Hello,
I saw the other issues this release has and ask if the problem I have is different or another issue the needs be addressed.

I added another table and need to insert another set of values... SO I used the insert single record from the link:

Server Extensions / WebAssist / DataAssist / Insert / Insert Single Record

This opens a new dialog forces me to create a session value unlike what I am used to where one is created or suggested for me. Then the page results in a fatal error which even Error reporting from PHP will not list.

The other or more usual way I would use the insert record extension is from this path:

Server Extensions / DataAssist / Insert / Insert Single Record

The dialog windows are different and also the first method inserts a new helper file:
<?php require_once("../webassist/database_management/wa_appbuilder_php.php"); ?>

The original method would insert this helper file:
<?php require_once("../WA_DataAssist/WA_AppBuilder_PHP.php"); ?>


Now both helper files exist, the page fails and I have to wonder if the updated methods also needed house cleaning to remove older versions of similar files??
