1) there are a number of possible reasons the table was not able to be created, usually this happens if the site definition does not have a testing server defined properly, or if the testing server is the remote server, not a local server.

The create table statement is saved to a .sql file so that you can create the database manually. Use the webassist/security_assist/site2_ps4_users.sql file that is saved to create the table.
