How to populate thumbnails into database

It has been a while since I did this and I can not remember (or figure out) how to populate the thumbnail name into the database after configuring the upload to create the thumbnail.

I have created a form with a total of seven images to upload. Those images are uploaded successfully and resized and renamed properly. The name of the primary photos are being populated into the database using

<?php echo $WA_DFP_UploadStatus["WA_UploadResult1"]["serverFileName"]; ?>

<?php echo $WA_DFP_UploadStatus["WA_UploadResult2"]["serverFileName"]; ?>

etc.

But I can not seem to get the thumbnail name loaded into the table. At best it populates the name of the main image in to the table.

There is no server entity in the dialogue box that corresponds to the thumbnail. If there is I can't read it because the dialogue box is so narrow.

I am on a Mac.
