what version of eCart are you using?
Are you using Universal Email 4 or are you using CSS Form Builder?
If you are using CSS Form Builder, make sure that you have installed CSS Form Builder 2.0.1 support release 2 that is available in the support release section of the CSS Form builder download center page, if you have a previous version installed, make sure to uninstall it from the extensions manager before installing the newer version.
after installing the Support release, delete the checkout pages and rerun the checkout wizard.
