UE Not sending from ecart page checkout.php

Hi,
During a checkout procedure, my client would like to find out where a customer heard of them, so I have added a text field in the form for them to complete before confirming their order. It does not get stored in a database anywhere, but triggers an email to be sent when the 'Checkout' button is pressed.

However, it doesn't work! I've added the UE trigger and email details in etc, but don't understand why it won't send.
