File Upload Problem. Has Worked Before. What am I Missing?

There has to be something simple here I'm missing. Attempting to add a file upload field like I have numorous times before. Have followed the "Adding Digital File Pro to DataAssist Insert and Update Pages" to a 'T' several times on this one, and still the file doesn't show up, nor does the DB fill the field properly. I'm uploading to the same directory as I am with another form, which still works properly. The entire insert form works fine prior to the upload manipulation, and seems to work fine after, but no file and no file inserted in the DB either.

The only thing different on this than the other insert on this site is the other one I was converting sizes and this one I'm not, since I want to be able to upload PDFs, DOCs, etc. and not images.

The only other thing I can come up with is the table field name is 'file' which ironically is also the input type. Could that be causing some kind of weird conflict?

If not, is there a list of steps to troubleshoot common issues here? I just can't figure out why it works fine on one insert page, and not this one.
