extension manager problems

i have a lot of the extensions listed in the extension manager, but i am not able to access them from the cs4 dw. The extensions are not found there, and there is no webassist dropdown either. Other extensions from other authors are also listed but not available. i tried to remove and reinstall, but i received an internal error message from the extension manager. I've rebooted as well. are there any remedies? suggestions?
thanks in advance
ps. i have also removed the winfilecache file.
update: i have now also removed the entire configuration folder, and replaced it with an old one.
this time i was able to install one of the extionsions - the surveyor - and it successfully installed, but i cannot again find it on the menu system, after restarting dw as well.
update 2: after removing and reinstalling i am able to work again...
