Forms Creating but not working since SA2 install
Installed SA2 and created a new form.
Uploaded form to website and the only thing that shows up when the form is viewed live is the{ class="groupHeader" Volunteer Form }. Here is the link to the live page:

I have uninstalled CSS Form builder 2 (replaced shared.js), Form Toolkit and SA2, then re-installed and deleted Dreamweaver cache. (FYI: I did uninstall SA1 prior to the initial installation of SA2)
Still same issue.
Started looking through the code of a working form and the newly created form and found that all of the php code above the <head> tag, as the sample code below, is missing.
Here is a sample line that is missing from the form that not working correctly:
$WAFV_Errors .= WAValidateRQ((isset($_POST["Clinic"])?$_POST["Clinic"]:"") . "",true,1);
I am figuring I can fix this by copying this code to the new form and replacing the new form $_Post array info with the proper information but it kind of defeats the purpose of having the extension and not having to write the code myself. (Not trying to be snide)
Running Mac OS 10.6 with Dreamweaver 5.5

UPDATE:
I did attempt to put the missing php code on the form page that is not working and it did not help. Now I am at a loose as to where to go now.
