How do I add a "textarea" form to a product page?

How do I add a form to a product page?

I would like to add a few "customizable" signs to a current site that is live.
THE GOAL, is to allow customers to add their own personalized text to a "SIGN" and send this with their order.

I need 1 field for an email address, domain name, or personal name, etc..
There are also signs already on the site that do not, and will not, require this customization.

Can you give me some direction how to finish the coding?
I already made an attempt at this at
I added a form "textarea" to the products_details page
I added a column to the ps4_orderdetails named "DetailPersonalize"
This is where I am having difficulties moving forward
What's next?

Any help would be greatly appreciated.
