Dynamic drop Down with multiple insert

Progress so far

1. I have created the individual recordsets for my parent and child table.
2. I have created menus for both parent and child and convereted them in dynamic menus using the created recordsets.
3. i have copied the code you pasted and pasted into dreamweaver and created a new include dependSelect.js
4. I am now calling jquery and the dependSelect.js in the <head> of my form page.

I will just show a generic example of what I am doing so you can help me based on this.

I must be missing something because I could not get it to work I am sure it has something to do with this code in the <head>. This is from your example but I would be grateful if you could show me how this works based on the generic form I have provided.
