Captcha not working :(

I have UE4 and the Form Toolkit and I'm working in DW CS3. I've watched the tutorial video and read a whole bunch of threads on this - but my form is not working.

These are the steps I took to build the form (in the order recommended by Jason):

1) Created a form in DW with 3 simple fields (with validation) and a submit button.
Form settings are: <form name="form1" method="post" action="">

2) Added captcha using Form Toolkit (following video tutorial)

3) Added validation using Form Toolkit (following video tutorial) - I used "like entry" and added text next to the Captcha text box "Invalid code, please try again."

4) Added the UE "Create Email" function

The form looks right, but no matter what I key in the CAPTCHA field the form simply refreshes and no email is sent out.
