Spam and New fields randomly generated?!

Sorry Mike, I can't see what you mean when you say 'bind form fields'.

Anyway, I've started again with a new form to see if it will work. However, even with the most simple/basic options entered, I still receive an email full of 'Lorem Ipsum' text that I haven't asked it to include (and can't see any way of removing it).

When I look at the form it creates in DWCS5, only the simple form fields appear to show.
Additionally, when I receive an email, there are categories marked as:

YOUR EMAIL TITLE GOES HERE
email subtitle area or directions can go here, below the title

As far as I can see, I haven't entered this information when creating the form and, once it has been created, it doesn't show in DWCS5 on the form page. So, I don't see how to remove it.

If the form was just for me, I could just about tolerate it but a client would ask me to remove it. And I don't fancy the conversation if I said it's impossible!

Any ideas why this doesn't work?
