Can you please make a suggestion on what I need to do to reproduce the error. We have experiencing the same issues every time we rebuild the site, and we have even tried making completely new sites (without bringing anything over from the old) and making a dummy index.php and building the cart again off of that - and we get these same issues.

Is it possible that I have too many extensions installed? Should I completely remove DreamWeaver and all extensions and try again?

Please let me know what I can do to help with this. We need to to work reliably, pretty bad. So We're pretty desperate to get this worked out.
