Captcha fail wipes out data?

I decided to start with a new form, and again, error after error. I've done it 4 times, same thing...see if you can recreate this problem:

create two blank PHP pages (HTML5 doctype):
default.php and send.php

Both pages are empty.

Insert CSS Form builder like this:
1) Contact Form - Simple Contact
2) Design - SHARP
3) Action page - send.php
4) Type POST

One time I did this, it didn't insert the include statements. Another time I did this, I get parse errors on the send.php page and nothing validates. These are the files I attached.

Keep in mind I did not modify anything on these pages - this is the code inserted by form builder.

I'm not PHP expert, but on the send.php page, all those calls to WAValidateRQ, WAValidateEM, WAValidateEM, WAValidateRX don't have semicolons after the statement - shouldn't they?

Another question - each time you insert another form somewhere on the same site, it updates the files in the WA_ValidationToolkit and SpryAssets you have to keep uploading these files each time?
