entry was simply "512" in the kilobytes field...a half mb basically. I have ruled that out as the problem however. it appears the "web images only" restriction was preventing the upload. The file was a .jpg, which is obviously an acceptable format, but for some reason that setting was preventing success. i'm still doing some testing to see if it's that particular .jpg, all jpg's, etc. I also set the dimensions so i'm making sure that's not the problem either.

can you help me with my other question about truncating the file name to enter the image location in the DB? thanks!
