Basically I've narrowed down the problem I was getting with that duplicate entry error message: when the form inserts to the database it's passing two values through as '0', rather than passing the correct values which should correspond to the id numbers so that the review is then linked to the correct practicename.
That's essentially what I posted last night to the DD forum - Dave Buchholz asked me to post the pages concerned (and I included the db as well); he posted last night that he'd look at them this morning (it's 22.24 here in the UK as I write this) but haven't seen a response yet from him. The latest situation is on DD forum.
