radio buttons, checkboxes required

Jason has helped me with this form in the past and was hoping for some help in finishing it up.

1. Spry Checkbox1 and Spry Checkbox2
There are two statements at the bottom of the form that need to be required. I haven't found documentation on how to make them required using this webassist form.
Please advise.

2. Spryradio1 and Spryradio2
I want the Yes to be checked by default, but if checked the textfield below showing the label "If yes, how many?" needs to be required. Is there an easy way to get the textfield to show when Yes is checked, and not to show when No is checked? I found some js scripts but none that worked with webassist.

3. In the checklists, is there an easy way to require that at least one checkbox will be checked before the form sends?

4. Lastly, is there a way for nonrequired fields, radio buttons and checkboxes to maintain their state when the form bounces back for further validation? What needs to be added?
