javascript error when click insert

Thank you so much for you expert help with this, As I said I had read all of the forum post about the fixes for the issue but at no time did I even think about a form that was in an include file. Boy I am really red faced now.

I do have to say that I learn quite a bit just watching you work while sharing my screen, such as going back to the basics and working forward from there.

  We looked into this issue in a connect meeting and found that there was an included file that contained three extra form in it, the first form had some text boxes that did not have the type attribute defined. The missing type attribute was causing the error.
