If the transactions were ran back to back it is most likely the session id that is the same and causing the order to be looked at as the same one.

To resolve this you should regenerate the session id on your checkout success page. The code that does this is:

php:
<?php

session_regenerate_id
();
?>



This is not a part of the default clear cart functionality, you must add this manually to regenerate the session id and have the transaction viewed as a new order when making back to back transactions.
