Adding an input form into shopping cart

I sort of got is working, thanks for the help. However, there's a couple things I would like to be different. I would like to have it be a couple dollars extra to customize, and right now you can customize and it doesn't add a price to the book. Maybe a check box that they check and then the customization appears (that might be too advanced, but anything that works). Also, when you add more than one book I would like to be able to have each one be customized individually just in case someone is sending multiple books to different people. Is that possible?
