Ok, so in the Create Email Message dialog box, you will need to click on the lightning bolt icon next to the From field. Then there should be an entry for the form and you can select the form field that is for the from email.

For adding the form bindings to the email itself, if you click on the edit with the pencil next to it, its easier to add the bindings. You would just click on the lightning bolt icon at the bottom of the interface and select the form field and hit ok. This will add code that will get the info from the form field and put it in the email. Make sure you add each of the form elements separately.
