Received email without contact informations

Email received with no contents

ok I set up a testing server on my site I get a menu that says Select Menu bindings and there's a 2 dropdown arrows: Appointment Request standout--under this server variables-- & Universal Email 2 (mail). I don't understand how this remedies my issue. I get emails in my inbox but they don't contain the info entered by the user--nor does the user get a CC'ed email of the form. my inbox says my email address also instead of the email address entered by the user. I understand that PHP is server side code so I would need to provide the PHP file to whoever assists me.
