Form submit problems

I am still having problem so I eliminated all but a few fields and created a new, fresh file. I uploaded it to my server and am using the e-mail address and smtp from the ISP. This is how the form will be returned once it starts working and goes into use. I an now able to receive an e-mail and have the submit button go to the re-directed page. The only thing I am not seeing is the form information. I have 2 forms. One with 2 spry widgets in template_form_startOver_022310_1500.php and template_form_startOver_022610_0900.php has the basic text fileds. I have uploaded the new file so you can see what I have done and why I'm not getting the information back. By the way. I did try sending with the suggestions that you sent earlier to no avail.
