Form submit problems

first off, remove the code from line 536 - 544:

the extra form tag may be getting in the way.

Second, the client side validation you applied to the form submit does not include the Show Validation Errors behavior. It is possible that form is not passing validation, but since the show validation errors behavior is not applied, you wont know it. either remove the validation, or add the show validation errors behavior.

third, the sample script that was provided uses the smtp server that is set by the php.ini file.

In Universal Email, try leaving the SMTP server field blank so it will use the same one as the sample script.

fourth, try leaving the from name blank.
