the best way of doing this is to update the quantity when the transaction is being processed. you will also need to add logic before the transaction is processed to check the quantity and make sure it has not been purchased by another user between adding it to the cart and checking out.

if you update the quantity as soon as the item is added to the cart, you would need to come up with logic to reset the quantity if they just leave your site and do not checkout, this is much more difficult
