I would like to help you get this resolved as quickly as possible. In order to do that though I will need some more details, I think if you can post back with the newly updated page in a zip archive this would help to clear up many of the questions that I have.

I will try to help you troubleshoot the issues on this page one at a time. I would like to start with the first set of changes that you have made to the form with CSS Form Builder. I will check on the alignment and the asterisk on the form and let you know what I think about getting these to the state you would like.

Once we can resolve those issues I will look into the uploading and the emailing problems you are experiencing. Just updating the form with CSS Form builder should not cause the emailing to break. As for the upload were you ever able to get this part successfully implemented on the contact form before you updated it with CSS Form Builder?
