Java Script Error

Java Script error(s)

I Install the extention and removed, I followed the instructions on removing the WinFileCachexxxx.dat file rename/ removed the confguration folder, tried reboots change order I did the instructions still I open site import on step one the wizard window opens then the Activation windows both are just blank boxes, then I get the following error window. (see attachment)

while executing onload in WA_Importsite_ser.htm the following javascript error(s) occurred:

Document.formLayer.document has no properties

This SiteImport 3.03, Dreamweaver CS4 on Vista
please help
