To Jaffa, if you are using a local checkout and having this issue you should login to the site and update your ticket to include additional details.
To John, you will not have the email option available to you because of the way that the remote checkouts work versus the local checkouts. During a local checkout the email receipt is sent after the user confirms the order and the status of the transaction comes back as successful. In a remote checkout situation it is not possible to confirm the status of the order when it is submitted, therefore sending out an email receipt would not be appropriate without first knowing this status.
For the shipping you must specify that you will be offering shipping in order to have the shipping part of the form included. If you do not wish to use the real time shipping quotes then you will just select none for the provider when configuring the shipping details in the wizard.
When it comes to the view cart code I will do some more investigating into this to see why this is not being defined on any of the pages.
For the orders info please explain the exact steps that you took up to this point before you clicked on back and I can check on some things to see what may be going on.
