How do I get the form to actually work?

In order to get your form to send the info in an email and store it in a db you would need to make use of two of our other tools, they are Universal Email and DataAssist. Universal Email will allow you to craft an email message to be sent back to the user, to you or any other email address you would like.

Using DataAssist will allow you to get the info stored into your db so that you can access it at a later time.
