Preferred Payment Gateway ? Ans: Paypal
Preferred Shipping Provider ? Ans: -- will be multiple, USPS, UPS and FedEx

Is Graphic Design Needed or is the Design to be supplied ? Ans: Graphics will be provided.
Are customer accounts (log in) required ? Ans: Yes -- most definitely.

Budget so that we don't waste our time writing an RFP if your budget is unreasonable.
