Captcha -- Not displaying correctly

Jason -- I have used UE once before and it worked very well. I did it by following the instructions\guideline on your website. But when I was setting up the second Contact Form -- it seems that there is a huge difference in the instruction guidelines from the one that I used on my first contact form.

Below -- please find the first contact form where I first used UE.

ljcontact100109.php <-- this form works w\o any problem.

Then the second one: contactus102909.php <-- this does not work.

I am a little frustrated -- At this point I will need to have someone contact me so I can get this thing to work.

You can see -- I need to get the contact for to work first before I re-arrange the site to look a bit professional. I am running way behind shedule.

Any help will be welcome.
