re: form not working

Okay, so I gave up trying to merge the forms following the latest instructions above. I couldn't get it to work. So now I'm trying to use the WA form and drop in just my form but with the security stuff at the end. I'm doing this because I don't know how to use Spry. Here's the page (it's not working): contactus.php. Am I "barking up the wrong tree", is this going to work?

Here's my regular, unmodified order form: order-form.php

This is very frustrating, to say the least, and is taking forever to get taken care of. I've had to go back to the old, unsecured order form for now because it's taking to long to fix the problems.
