That should insert a comma separated list of states selected into the database.

While waiting for your response I took time to check other related questions and got an idea from one of them and then change my state code line
from
$InsertQuery->bindColumn("state", "s", "".((isset($_POST["state"]))?$_POST["state"]:"") ."", "WA_DEFAULT");

to
$InsertQuery->bindColumn("state", "s", "".((isset($_POST["state"]))?implode(", ",$_POST["state"]):"") ."", "WA_DEFAULT");

and that solve the problem.
