Like I guessed, the issue with the venurRegister.php page is that the <form> tag doesn't have enctype="multipart/form-data".

Did you copy and paste on the register_exec2img.php page? The interface should uniquely name the objects, but yours both have the same name. That is why it is showing the same file.

Update every reference to: WA_UploadResult2

on lines 34-53 and change it to: WA_UploadResult1

Then you will have two uploads instead of overwriting one with the other.
