This is a rather complex page and it appears to have several issues. I see that you have several form elements all with the same name="submit".

Names for form elements have to be unique. The name is what is used in the php code to refer to the value submitted. If they have the same name, then you can't differentiate one from the other.

The first thing you will need to do is make sure all of your form element names are unique and then update the recordsets and inserts to refer to the correct form elements.

The next issue I see is that you have 3 recordsets on the page all named "Recordset1"... Recordset names also have to be unique.

I think renaming the form elements and recordsets are a good first step. Then update the recordsets and insert server behavior to reflect the updated form element names.

If you still have issues after that, then send me the updated file and I'll see if I can spot any additional issues at that point.
