Submit buttons not displaying

Did you download the updated page that I put up? Dreamweaver should never update html code when you save. My best guess is that you didn't download my changes so your saving your local copy reverted back to it. I'll upload my version again. Please download it and you can note the difference.

The form fields in the working version look like:

The original version had a blank value tag and missing code to open the <?php tag so that the php code is displayed on the page.
