Which shipping are you using? What books and address should I use to reproduce? What shipping should it be charging? It looks like you have a bunch of different shipping rules... I'll need more details to be sure I'm looking at the right thing. Please tell me:

1) what products to add to the cart
2) what shipping address to use
3) how much shipping you would expect to be charged (what shipping rule should be used in this case that isn't being added)
