Stumped by tutorial on how to create a file upload using DataAssist

The Prod_Lube_testimonials table isn't receiving the correct IDs in three columns. 'LubeTestim_ID' and 'LubeFK_ID' both have '0' values, and 'TestCom_ID' has a numbered value, but it's not correct.

LubeTestim_ID should get its value from the Testimonials table (testim_ID) when the new testimonial is uploaded.
LubeFK_ID should get its value from the Testimonial Lube radio button
TestCom_ID should get its value from the Testimonials_company table (TestCo_ID) when the testimonial company is entered in the form field.

Have tried binding to session variables, form fields and recordsets, but none insert the proper values.

Also, the form uses two checkboxes (Lube Type and Industry - each with their own ‘insert multiple records’ server behaviors). They both insert values into the 'prodtype' table. When one item is clicked on each checkbox, the result is two new records. Would like to get these two values into one record, so when multiple items are clicked the number of records is easier to manage. It was my understanding each checkbox needed its own ‘IMR’ server behavior.
