It looks like all of your insert values are bound to recordsets. That seems wrong. It seems like you would only have two bound fields...

1) The inserted id, which should be the session variable that you are saving the inserted record as: <?php echo($_SESSION["WADA_Insert_prod"]); ?>
2) The checkbox value which should come from the repeated field.

Only one repeated record is showing in your UI because it appears you have copied and pasted to create the three repeated regions instead of applying it from the server behavior each time. That resulted in you only having one repeated region recognizable by the interface. I deleted the second two and applied them from the server behaviors panel instead, so they should show up correctly now.

The checkboxes should also be created with a value attribute set to the id that you want inserted into your child table. It looks like your checkboxes have no values. I've gone in and corrected that as well.

Please download the file and you should be able to update the Multiple insert server behavior bindings correctly now and set the two values that need to be set.

Did this help? Tips are appreciated...
