Form Builder form won't send email

I've added a 'commission' checkbox and 'resume' File Manager upload elements to the form, with validation for the checkbox. Followed the WA tutorials for the file upload and UE email attachment, though my UE dialog doesn't show the same items as the tutorial. Sent the updated form about 10 am CST this morning, but still haven't received an email. It took about 4 hours yesterday to receive an email without the added form elements.

The bindings on Blank_2.php for the persistent form show all the form elements except for the upload and checkbox; not sure why.

1) Don't know why the form is either not sending, or I'm not receiving. The other forms on the site work fine.
2) Why is it taking so long to receive the email? The other forms on the site don't take that long.
