Search page pulling in incorrect data

Thanks. I am going to give the guy and option to pay for a premium ticket and have you look at that part.
If he does not want that I will just do a single multi-level dropdown so they will only be able to choose one thing.

On this page: http://4patientsafety.org/mdb/search.php searching from any of the items work except for the infractions drop downs.
If you enter ADDY in the Infractions text search field (which is only temporary just to see if it found any results) it finds the result that it Record #1.
It does not list the infractions on the results page and that is intentional. If you go to the detail page it shows it (not the way I want it to look but it is there).
Now if you go to the Practice drop down and choose the top item (Failure to report change of address, which is ADDY) it finds nothing.
The table it is using for the dropdown is infrCodesPractice with the columns of infrCodesPracticeID (numeric), infrCodesPracticeAbbr ( 3 or 4 alphanumeric letters), and infrCodesPracticeDesc (long description of each code).
I set the 'set value equal to' in the dynamic drop down setup to the same thing the the infractions text field is.

I cant understand why it is not working for at least one of the drop downs. I can imagine it could cause problems if more than one was selected.
I dont know if it is possible but limiting the choice to only 1 of the 6 dropdowns would also be fine.

Then on the detail page after it finds something, I would like it to display the description not the 4 character code.
