How to add validation to select list in DataAssist form

I'm afraid I can't identify what to change based on your answer. Are both the matching identifiers in the sample code below, or are there others somewhere else?

Also, the WA Server Validation server behavior for Email listed in the wizard looks different than the others; doesn't have the 'required / not blank' statement. Not sure why or if this is a problem. I also have three 'what if' server behaviors that reference the Email field, and one that not only lists all the fields, but duplicates them as well. I tried deleting the duplicates but they won't delete.

Lastly, much of the code references findsalesrep.php which doesn't exist anymore (changed to ContactUs.php). Can I change all the old references to the new page or will this cause massive malfunction? It doesn't seem to affect the page working.
