I was doing lot of test & i found that the problem is with input checkbox name.
when there are two records displaying with checkbox its input seems like this for 1st record:

<input name="Emp_0" type="checkbox" id="Emp_0" value="6">

and 2nd:

<input name="Emp_2" type="checkbox" id="Emp_2" value="5">


2nd should be Emp_1 as 1 gets added all time as i noticed. but here its 2 been added. So when i changed the value from inspect then submitted seems all works.
Also hidden input also adding 2 rather than 1.

<input type="hidden" name="Emp_mihidden_2" id="Emp_mihidden_2" value="1">


So what can be done here? Why is 2 been added? Latest file attached.
