create a folder on server when a form is submitted

Our web-site has a members area where members login and can submit a case/enquiry via a form. the form has an 'upload file' behaviour so they can attach any documents that assist the enquiry. currently all files uploaded by members all go into one folder. What I would like is for each member to have their own sub-folder created using the members 'primary key' as the folder name. If this is possible, my next question is can this be done from the same form submission with the file attached or will the folder have to be created first with another action. In which case I could have the folder creation happen when the members first submit their application form. Hope this makes sense!
