This was a site settings issue. If it appears your extension isn't connecting to the database, go to the database tab and test the connection. If Dreamweaver can't connect to the database, then neither will the server behavior.
There are a few things to look for if your connection doesn't test properly:
1) Check your site settings. Dreamweaver uses whichever server is checked as "testing" when connecting to the database
2) Make sure your site url is correct. Browse to your web site and copy and paste the url from the browser to make sure you didn't make a mistake and that there is no redirecting going on.
3) Make sure your username and password are correct for the server you have checked as the testing server
We corrected these issues on the phone and once Dreamweaver could connect to the database the extension started working as expected.
