DataAssist: Insert single record -> General -> database -> table : ***No Table Found

I tried everything and it still does not work. I even deleted all files and started from scratch.

I can only get to step 2 of the "Add the string to the database" section of the Double Opt In Registration tutorial. When the DataAssist: Insert Single Record server behaviour popup appears, the users table does not show up even though in the Insert php script $WA_table = "users";. So, of course, I cannot continue to the next step to choose a column from the Bindings tab as there are no column appearing.

I should say that I already tried all your suggestions by
1) deleting the redirect manually by setting $WA_redirectURL = "";
2) making sure the localhost and remote servers were configured correctly as per the Define a dynamic site in CS5+ tutorial
3) the database connections are configured correctly also
4) I even tried reinstalling the DataBridge extension

This leads me to believe the extension code itself is flawed and not picking the users table. What is the manual work around?
