Adding Category Name to Page Title and Custom Meta Description

Trying to add Category Name to the Page Title on the Products Results page so that the name of the category shows up in the title. I was able to do something similar with the Products Details page by making it show the Product Name in the Page Title. Now, the Page Title on the Products Results page is only showing the Category named "Dinnerware" which is CatID 1, no matter which CatID it is going to. The pages are throughout my products, one example is:, which pulls products under Category 5 with the Category Name "Chairs", however you will see the Page Title says "Dinnerware". How can I fix this?

I would also like to have custom Meta Descriptions for each products results page. How can I do that?
